• Clinic includes day of clinic activities, porch visit from cheerleaders prior to the clinic, poms, t-shirt, and a pre-game experience at a Varsity Home Football Game.

All participants need to fill out this permission form before the clinic.

Clinic will be held in the PHS Arena, on the Gymnasium side of the building.
